Overview

Think partnered with Nemours Children’s Health to make pediatric care as accessible as everything else in a parent’s pocket. Nemours is one of the nation’s largest multistate pediatric health systems, with two children’s hospitals and more than 70 specialty practices, answering families’ most urgent questions every day. Before designing anything, we conducted 32 interviews across four Nemours locations, talking with clinicians, nurses, and parents to understand what was breaking down in chronic care. That research shaped a four-year digital roadmap, pointing to one proof-of-concept worth designing first: a telemedicine app connecting families managing childhood asthma directly to their care team. That single pilot proved itself well beyond asthma. The app now supports families managing heart conditions, diabetes, and epilepsy, syncing data straight into Epic so physicians see the full picture before every visit. Nemours brought Think back through each new phase. The app is now rated 4.9 out of 5 by nearly eight thousand families.
